Instructions for Students

To have an academic coursework exam proctored offsite please follow these instructions:

1.  Complete the top section of this form.
2. Contact a college or university testing center near you to see if they are willing to proctor your UT test and what they will charge. 
3. Send the form to your local proctor so they can complete the bottom portion and they will send the completed form to the UT Testing Center.
4. We will send your proctor the test information after your Professor provides it to us.
5. Check with your proctor before test day to ensure they have received the test information.
